HCRS (Health Care & Rehabilitation Services of Southeastern Vermont)•Brattleboro, VT
•$22 - $24

About The Position

Healthworks Assertive Community Treatment (ACT) is a participant-led approach to improving care and outcomes for people experiencing homelessness or housing insecurity in Windham County. It is an intensive, integrated services and support model for people whose basic needs are not being met by existing housing, medical, and mental health services. The team provides services chosen by participants, helps them progress toward their goals, and adjusts support as their needs change. The staff-to-participant ratio is small, and services are available 24 hours a day, seven days a week, as needed. Team members provide outreach to people enrolled in ACT and/or served by Groundworks, including in community settings such as shelters, tent sites, and hotels. The team advocates for participants’ access to and autonomy in physical and mental health care through an equity and justice lens. The ACT Peer Support Advocate provides ongoing individual and/or group peer support using the Intentional Peer Support model, a framework focused on mutual relationships, connection, and learning together. The advocate helps participants build peer networks and natural and social supports, supports their participation in decisions about their lives and care, and advocates for their expressed interests. This position also contributes to the development, implementation, and maintenance of ACT team services and collaborates with other team members to provide comprehensive support.
