HCRS Current Positions•Brattleboro, VT

About The Position

Healthworks Assertive Community Treatment (ACT) is a participant-led approach to improving care and outcomes for people experiencing homelessness or housing insecurity in Windham County. It is an intensive, integrated services and support model for people whose basic needs are not being met by existing housing, medical, and mental health services. The team delivers integrated services chosen by participants, helps them make progress toward their goals, and adjusts services to meet changing needs. The team maintains a small staff-to-participant ratio and provides services 24 hours a day, seven days a week, as needed. Team members conduct outreach to people enrolled in ACT and/or served by Groundworks, including in community settings such as shelters, tent sites, and hotels. The team values and advocates for physical and mental health access and autonomy through an equity and justice lens. The Peer Support Advocate provides ongoing individual and/or group peer support to participants who want these services, using the Intentional Peer Support model. The role supports mutual relationships focused on connection and learning together, helps develop and maintain ACT services and peer support networks, and advocates for participants' expressed interests. The advocate works with team members to ensure participants can take part in decisions about their lives and care to the extent they choose.
