RainCity Housing is an organization that makes a difference in people’s lives. Our mission is to advance social equity by building upon the strengths of our community and of each individual. With a goal of providing a home for everyone, RainCity’s programs sustain relationships, strengthen communities, and make change for people experiencing homelessness, mental health, trauma, and substance use issues. RainCity Housing has been developing emerging and promising practices for over 40 years in social services and healthcare, providing Housing First, harm reduction and supports for community members who access our services. ACT nurses assess, identity, plan, implement and evaluate the care required to assist ACT team clients in meeting their physical, psychiatric, social and spiritual needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client centered and strengths-based care in accordance with the BCCNM Standards of Practice and consistent with the mission, vision and values and established policy and protocols of RainCity Housing and the ACT team. Many of our clients are certified under the Mental Health Act and knowledge of the Act and its implications and limitations is essential to working as an ACT nurse. The Nurse provides treatment services that are both inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions and ensures integration and continuity across the care continuum. The Nurse establishes and maintains relationships with, and consults with community health clinics, hospitals, housing providers and the interdisciplinary teams to provide appropriate treatment services based on best practices and client need. ACT is an evidence-based transdisciplinary mental health team that provides outreach support to clients with severe and persistent mental health and substance use diagnoses. The RainCity ACT team also provides housing support for clients using a ‘housing first’ framework. Self-determination, choice and harm reduction are at the centre of all considerations with respect to the provision of ACT services.
