About The Position

This position is responsible for intensive case management services as identified by the ACT (Assertive Community Treatment) clinical team for individuals diagnosed with moderate to severe mental illness and co-occurring disorders.

Requirements

  • Bachelor's degree in human/social service field
  • 3 years working with SMD population
  • First Aid certification
  • CPR certification
  • De-escalation certification
  • Valid Ohio driver’s license
  • Proof of automobile liability insurance
  • Meets OMHAS licensure and certification requirements
  • Competencies for serving the SMD population
  • Knowledge of safety rules and procedures, agency policies and procedures, Greene County area and resources, documentation requirements.
  • Skills in driving, computer literacy, interpersonal skills.
  • Capacity to perform duties effectively under potential emotional stress and conflict situations.
  • Proficiency in utilizing basic computer skills and office equipment for documentation and communication purposes.
  • Practice of a client-focused approach, demonstrating active listening skills, reading comprehension, critical thinking, and leadership abilities.
  • Ability to lift up to 40 pounds throughout full range of motion.
  • Ability to use of computer keyboard.
  • Ability to sit for periods of time.
  • Ability to see, hear, and communicate information clearly.
  • Ability to recognize unusual or threatening conditions related to persons with disabilities.
  • Willingness and physical capability to engage in activities are essential to meet the fundamental needs of individuals served.
  • Occasional assistance in relocating individuals to new living situations, aiding with shopping, and instructing daily living skills, as necessary.

Responsibilities

  • Provide individual case management under clinical supervision, meeting productivity and quality requirements set by the Agency and within the limits of services permitted by position and scope of practice.
  • Accumulate the required number of billable hours of clinically allocated time based on Agency standards for position and profession.
  • Ensure ACT Case Manager-canceled appointment rate does not exceed 10%.
  • Meet or exceed standards pertaining to assessments and documentation.
  • Identify Individualized Service Plan supportive services authorized by Utilization Management and provide linkage to these services within authorized duration and frequency, including working with Mental Health and Alcohol and Other Drug clinicians to develop multi-disciplinary services to ensure continuity of care.
  • Ensure each progress note demonstrates client progress and logical case management action steps.
  • Ensure progress notes and other documentation clearly identify consumer and ACT Team member input services.
  • Complete progress notes at the end of each session and within 24 hours of completion.
  • Develop and maintain quality, accurate clinical charts for all clients served.
  • Develop and maintain positive collaboration with other social service agencies serving clients.
  • Maintain knowledge and familiarity with referral sources within the community and provide consultation when needed.
  • Serve as a liaison between the hospital, criminal justice system, and other treatment services in the community.
  • Maintain attendance at assigned clinical and administrative committee meetings and training.
  • Maintain license or registration appropriate to the profession and job requirements, including the appropriate amount of continuing education at all times of employment.
  • Complete agency-required annual training including client rights and grievances, safety training, and infection control.
  • Remain free from citations by other providers or consumers for unethical practice and meet professional standards.
  • Adhere to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
  • Participate in at least 6 hours of supervision for every 3-month period or meet the supervision requirements of the license/registration (whichever is greater).
  • Perform other clinical duties as assigned.
