About The Position

The ACT Case Manager is responsible for conducting needs assessments, formulating Individualized Service Plans (ISPs), and providing direct therapeutic interventions. This role involves crisis stabilization, assisting clients with Activities of Daily Living (ADLs), coaching, teaching, and gathering client feedback. The Case Manager will make decisions regarding clients' needs for additional services, resources, or more restrictive settings, and will choose appropriate treatment interventions and advocacy activities. They will also pursue collateral contacts for psychosocial information, adhere to risk assessment and management protocols, and complete various intake and discharge paperwork. The position requires interaction with various stakeholders, including peers, supervisors, other service providers, family, and community members, to advance treatment progress and agency standing. Active participation in agency meetings and supervision is expected, along with introducing new ideas for improving care quality and responding to feedback. The role also involves sharing knowledge with colleagues and pursuing continuing education. A significant portion of the role (75%) involves driving clients to appointments in a company vehicle.

Requirements

  • High School diploma required.
  • 3 years mental health/ SUD or community support/case management experience required.
  • Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
  • Knowledge of community resources is critical.
  • CDCA is required if a candidate does not have a Bachelor’s degree; candidate will need to have prior to hire or will need to obtain within 45 days of hire.
  • Experience using computers, Electronic Health Records (Credible), office phone and cell phone, email, fax/copy machine.

Responsibilities

  • Conduct needs assessments and formulate Individualized Service Plans (ISPs).

  • Gather client feedback.
  • Make decisions regarding clients' need for additional services, resources, or more restrictive settings.
  • Determine choice of treatment interventions, techniques, advocacy activity, and frequency of contacts.
  • Pursue collateral contacts to gather additional psychosocial information.
  • Adhere to agency’s risk assessment/management protocols.
  • Complete intake/needs assessment, ISP and reviews, progress notes, Payeeship forms, and discharge paperwork.
  • Conform to Program, ODMH, CARF and third-party payer standards and obligations.
  • Interact constructively with essential others to advance progress in treatment, quality of care, and agency standing.
  • Participate actively and productively in agency meetings and supervision sessions.
  • Introduce and/or implement new ideas for improving the quality of care.
  • Be attentive/responsive to feedback from clients, significant others, community partners/members.
  • Share skills, knowledge, and expertise with other agency staff.
  • Advance own level of skill and competency through participation in continuing education opportunities.
  • Utilize knowledge of community resources.
  • Drive clients to appointments in a company vehicle (75% travel).
