About The Position

Eliot is seeking an experienced, licensed RN or LPN to join its Adult Community Clinical Services (ACCS) team. ACCS provides clinically focused, evidence-based interventions to adults with severe and persistent mental illness. Services are person-centered and delivered by a multidisciplinary team in residential or community-based settings. The ideal candidate has knowledge and experience in mental health, nutrition, and medical care, along with interpersonal skills that support sustained client engagement and participation in health care and wellness.

Requirements

• Associate degree in nursing.
• Current RN or LPN nursing license; the posting also specifically requires a current RN license from the Commonwealth of Massachusetts.
• Valid Massachusetts driver’s license.
• Experience assisting individuals with mental health and wellness needs.
• Knowledge and experience in mental health, nutrition, and medical care.
• Strong interpersonal skills that support sustained client engagement and participation in health care.

Responsibilities

• Work with the multidisciplinary team to provide education about medical health, medications, and related topics.
• Develop and conduct health and wellness training for staff and clients, including topics such as nutrition and physical activity.
• Help identify and access community resources that support clients’ needs and wellness efforts.
• Guide and support clients in understanding their medical conditions.
• Meet with clients and staff to discuss medical diagnoses, including causes, symptoms, complications, treatment options, and prevention.
• Provide direct medical care to clients, including monitoring vital signs and providing wound care as needed.
• Review medical documentation for assigned clients.
• Assist clients and staff in assessing symptoms and determining appropriate responses or actions.
