The Leader We're Looking For We are seeking a strategic, detail-oriented Access & Outcomes Manager to lead intake, person-centered planning, outcomes management, and electronic health record (EHR) processes that ensure high-quality, compliant, and accessible services. This is a role for a systems-minded, data-driven leader who understands how strong processes, accurate data, and thoughtful planning come together to create better outcomes for the people we support. This role also champions human rights—advocating for dignity, inclusion, and ethical service delivery across all programs. This position has the ability to offer a hybrid work environment, however, to best support our teams and the people we serve, candidates must live within a reasonable distance of Waterloo, Iowa, to regularly work on-site as needed.

Requirements

  • Bachelor’s degree in human services, human resources, business, or related field preferred
  • Four (4) years’ experience in Quality Improvement, Information Management, and Administrative Support with a strong preference for functional databases and data analysis/reporting
  • Demonstrated leadership and ability to collaborate effectively within a team environment.
  • One (1) year of supervisory experience is preferred
  • Prior experience in delivery of services specific to intellectual/developmental disabilities, mental illness, brain injury or similar experience
  • An equivalent combination of education and experience will be considered
  • Strong verbal and written English communication skills
  • Proficient with computers and able to quickly learn new systems and technologies.
  • Valid driver’s license, reliable transportation, and insurance meeting agency requirements
  • Ability to pass required background checks

Responsibilities

  • Lead and oversee intake, admissions, and service access processes, ensuring timely, coordinated, and appropriate entry into services
  • Ensure Individual Service Plans (ISPs) are person-centered, compliant, and aligned with assessed needs and desired outcomes
  • Champion documentation quality and planning consistency across programs, ensuring services are individualized and outcome-driven
  • Monitor outcomes data, documentation quality, and reporting to identify trends and drive continuous quality improvement
  • Utilize data, reporting, and system insights to support informed decision-making and strengthen service delivery
  • Administer and support the Electronic Health Record (EHR) system, ensuring data integrity, usability, and compliance
  • Supervise, coach, and develop team members, fostering a culture of accountability, collaboration, and growth
  • Ensure all practices meet regulatory, accreditation, and compliance standards
