About The Position

The Access Navigator plays a critical role in guiding individuals through complex systems to ensure they receive timely and appropriate access to services and resources. This position focuses on understanding client needs, identifying barriers to access, and coordinating with multiple stakeholders to facilitate solutions. The Access Navigator acts as a liaison between clients and service providers, advocating for equitable access and helping to streamline processes. Success in this role results in improved client satisfaction, reduced delays, and enhanced overall service delivery. Ultimately, the Access Navigator contributes to creating a more inclusive and accessible environment for all individuals seeking assistance.

Requirements

  • High school diploma or equivalent; associate or bachelor’s degree preferred.
  • Experience in case management, social services, healthcare navigation, or a related field.
  • Strong communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Proficiency with basic computer applications and data entry.
  • Strong communication skills daily to clearly explain complex information and build trust with clients from diverse backgrounds.
  • Organizational skills are essential for managing caseloads, tracking referrals, and ensuring timely follow-up.
  • Problem-solving abilities help navigate systemic barriers and identify creative solutions to access challenges.
  • Collaboration skills enable effective coordination with internal teams and external partners to streamline service delivery.
  • Proficiency with technology supports accurate documentation and efficient management of client information.

Nice To Haves

  • Bachelor’s degree in social work, public health, or related discipline.
  • Experience working with diverse populations and understanding of cultural competency.
  • Familiarity with healthcare systems, insurance processes, or community resource networks.
  • Certification in patient navigation or related professional training.
  • Bilingual abilities or proficiency in a second language.

Responsibilities

  • Assess client needs and eligibility for various programs and services.
  • Provide clear information and guidance to clients regarding available resources and access procedures.
  • Coordinate with internal teams and external partners to facilitate service delivery and resolve access issues.
  • Maintain accurate records of client interactions, referrals, and outcomes.
  • Advocate on behalf of clients to overcome systemic barriers and ensure equitable access.
  • Monitor and follow up on client progress to ensure successful navigation through service systems.
  • Participate in continuous improvement initiatives to enhance access processes and client experience.
