About The Position

The Access Health Education Specialist is responsible for promoting health equity through community education, engagement, and care navigation services. This role focuses on connecting individuals to HIV/STI prevention, testing, treatment, and support services while addressing barriers to care through culturally responsive health education and advocacy. The Specialist works closely with community members, clinical staff, and partner organizations to advance public health outcomes and improve access to essential health services. Position is Grant-funded : Contingent upon continuation of grant funding from year to year.

Requirements

  • Bachelor’s degree in Public Health, Health Education, Social Work, or a related field required; equivalent experience may be considered.
  • Minimum of 2 years of experience in community health, HIV/STI prevention, case navigation, or related field.
  • Knowledge of HIV/STI prevention, PrEP/PEP, and health education principles.
  • Experience in reaching underserved and hard-to-reach populations and connecting individuals to appropriate services.
  • Proficiency in data entry, Electronic Health Record (EHR) systems and Microsoft Office.
  • Required field about 75% of the time and take mass transit to travel throughout New York City.
  • Demonstrated ability to work independently, and document all work performed.
  • Mission focus and have an understanding of the community’s cultures and ethnicities, and knowledge of community assets and resources necessary to effectively respond to needs and gaps in service delivery.
  • Demonstrate resourcefulness with proven ability to anticipate needs, prioritize responsibilities, manage multiple priorities with tight timelines, and take initiative.
  • Have a sensitivity to the needs and situations of a multi-cultural population with a variety of income levels.
  • Knowledge of Brooklyn-based systems of care.
  • Strong communication skills are essential.
  • Demonstrated ability to conduct sensitive, empathetic interviews and conversations that highly respect the dignity and diversity of clients.
  • Ability to engage in a supportive and empowering way.
  • Ability to help manage conflict.
  • Ability to maintain client records and information in an accurate, timely and confidential manner.

  • Ability to travel within the boroughs (automobile not required).

Responsibilities

  • Develop community-level partnerships to identify individuals who are underserved, uninsured, out-of-care, who are of the immigrant population and other hard-to-reach populations, including those in need of HIV care, behavioral health support, and chronic disease management.
  • Conduct outreach events, meetings and other activities at various community organizations and health information settings to raise awareness about health and insurance coverage, and to provide information about the services at the health center and its supporting programs.
  • Conduct presentations during various Brooklyn-based events and outreach settings.
  • Utilize social marketing resources to identify and contact hard to reach individuals who are underserved and underinsured.
  • Maintain an outreach database developed through regular contact with other community-based organizations, both HIV-specific and non HIV-specific.
  • Support the BSFHC HIV, PrEP and HEP C programs by referring partners and community members to available prevention services, particularly individuals identifying as MSM of color.
  • Provide individualized support to clients in accessing HIV/STI testing, PrEP/PEP services, primary care, and behavioral health resources.
  • Assist clients with appointment scheduling, insurance navigation, and adherence to care plans.
  • Conduct follow-up to ensure successful linkage, retention, and adherence to care plans.
  • Maintain confidentiality and client trust throughout the care continuum.
  • Deliver one-on-one and group-based health education sessions focused on HIV/STI prevention, sexual wellness, harm reduction, and other relevant public health topics.
  • Support the implementation of evidence-based prevention interventions.
  • Empower clients with knowledge and skills to make informed health decisions.
  • Submit clear and accurate reports based on data collection, outreach activities, and patient engagement across all service levels.
  • Keep supervisor informed of all problems, challenges and conflicts related to job responsibilities.
  • Work collaboratively with internal departments and external partners to coordinate services and improve access to care.
  • Participate in multidisciplinary team meetings and contribute to program improvement discussions.
