The Access Case Manager serves as a clinical representative and educator for Shepherd to external referral sources and evaluates external patients for appropriateness of admission to Shepherd. This role works directly with patients and their families, creating an engaging and dynamic work environment. The Access Case Manager helps to improve the patient experience at Shepherd Center. This position is primarily a face-to-face role that requires meeting with prospective patients, families, and hospital staff. In situations when it is not feasible for travel (e.g., driving distances over 5 hours each way, inclement or actual unsafe weather conditions, or requests to not go on site), these cases require approval from the CAM/Director. The Access Case Manager represents Shepherd to the external community, establishes and maintains rapport with families and referral sources, and participates with the marketing department to assist with referral development and managing account activities using the Salesforce CRM platform as outlined in the individual marketing plan. The role involves screening all medical records to ensure information is useful and in chronological order, assisting the Admissions Department with follow-up on clinical verification when there are gaps in time from referral, and screening orders externally and from SC clinics to ensure appropriateness of referrals. The Access Case Manager also determines if any Pathways OP referrals can be converted to Day Program referrals if appropriate and space allows, and determines if a patient does not meet admission criteria, referring them to alternate programs based on a telephone interview.
