The 1915(i) Waiver Care Coordinator is responsible for providing proactive intervention and coordination of care to eligible Vaya Health members to ensure they receive appropriate assessment and services. This role involves coordinating care for individuals eligible for 1915i services by the North Carolina Department of Health and Human Services (DHHS). The Care Coordinator works with the member and care team to address care gaps, coordinate multidisciplinary care planning, and facilitate linkage to services across the 1915i service array and other healthcare networks, including mental health, substance use, intellectual/developmental disability, traumatic brain injury, physical health, pharmacy, long-term services and supports, and unmet health-related resource needs. The position also supports transition planning assistance from facility settings and tracks individuals to ensure follow-up with aftercare services to prevent re-hospitalization. This is a mobile position requiring work in various locations, including members' homes, and involves collaboration with Vaya staff, members, caregivers, providers, and community stakeholders. Essential job functions include utilizing Vaya’s Care Management software, outreach and engagement, HIPAA compliance, performing the NC Medicaid 1915i Assessment tool, adhering to medication and continuity of care processes, participating in care team meetings and planning, transitional care management, and diversion from institutional placement. The position requires meeting NC Residency requirements.
