About The Position

The YouthBuild BRIDGE Case Manager--LCSW will provide specialized services to participants in the YouthBuild BRIDGE program. This is a three year grant with potential for renewal. The LCSW services will include facilitating educational opportunities and addressing participants’ emotional, financial, and community needs, while modeling best practices in psychosocial care for participants’ future endeavors in academia or the skilled workforce. Participants are placed in a cohort model with four cohorts and approximately 17 participants in each cohort.

Requirements

  • A Master of Social Work Degree from a program accredited by the Council on Social Work Education
  • LCSW in the state of Arkansas

Responsibilities

  • Provide specialized services to participants in the YouthBuild BRIDGE program.
  • Facilitate educational opportunities.
  • Address participants’ emotional, financial, and community needs.
  • Model best practices in psychosocial care for participants’ future endeavors in academia or the skilled workforce.
  • Conduct eligibility verification and develop Individual Service Strategies.
  • Coordinate supportive services.
  • Track measurable skill gains and credential attainment.
  • Maintain follow-up contact and retention documentation.
  • Facilitate referral to Adult Education and Career Pathways services.
  • Provide psychosocial support to participants.
  • Complete clinical assessments as per program standards.
  • In collaboration with a multidisciplinary team, develop and implement participant handbook; select candidates; and connect participants to services available at UA-PTC or community.
  • Provide psychoeducation on burnout.
  • Help participants navigate their own unique pressures.
  • Provide group therapy- clinical and psycho-educational.
  • Conduct research and assessment of program and participant successes.
  • Work with performance and test anxiety.
  • Provide referrals for learning disability testing.
  • Provide substance use therapy and education.
  • Address food, housing, and transportation insecurities.
  • Assist with job or registered apprenticeship interview preparation.
  • Discuss with participants their actual needs and collaborate with steering committee to meet these needs.
  • Be responsible for providing a full range of social work services including psychosocial assessment, treatment referral and resource referral(s).
  • Works collaboratively with participants, interdisciplinary team members, community resources and agencies.
  • Communicates timely, relevant and accurate information to all parties involved with participants’ matriculation through the program.
  • Integrates the work of all parties, resources, and services necessary to accomplish agreed-upon goals.
  • Advocates on behalf of the participants and identifies needed services and resources.
  • Assures appropriate utilization of all resources to achieve optimal outcomes.
  • Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice.
  • Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope.
