The Youth Tracker is responsible for completing assigned daily contact with youth and meeting the conditions of the youth’s service plan for their assigned caseload. This role requires a flexible work schedule, which may include evenings, weekends, and holidays. Key duties involve conducting face-to-face daily contacts at various locations, managing youth behaviors through programmatic and individual structure, and developing helping relationships that emphasize youth accountability. The Youth Tracker must complete all required documentation, including discharge information, accurately and in a timely manner, and promptly enter data into the Department of Juvenile Justice’s Juvenile Tracking System (JTS) and Southwest Key Programs database, Efforts to Outcome (ETO). They are also expected to familiarize themselves with youth history by examining case files and to notify the Program Director immediately of any youth crisis, suspected illegal activity, or other incidents. Regular and punctual attendance is essential. Additional responsibilities include assisting with youth activities and groups, providing staff relief and being on call, advocating for youth in areas such as health, education, vocation, legal, and family work, assisting with on-the-job training for new staff, and providing crisis intervention and first aid when necessary. The position requires the ability to react productively to change and handle other assigned essential tasks.
