About The Position

New Directions is a social service agency offering a wide range of resources and services that foster people’s hopes and dreams in their communities. The Youth Supported Independent Living Program offers supports for youth who have developed more basic skills to live independently with supports and are interested in starting to live more independently as they continue working towards their adult life goals (i.e., work, education, relationships, self-care, life in community, etc.), and solidify their independent living skills (i.e., fully manage their budget, keep up with work and education responsibilities independently, use community resources, take care of their mental and physical health, connect with significant others and community meaningfully).

Requirements

  • Applicants must have a B.S.W./B.A. or related social services diploma with experience working alongside individuals who may have intellectual disabilities and/or experiencing mental health barriers.
  • Applicants must have a minimum of 5 years of experience providing case management or support within the child welfare sector or a related field
  • Experience with referral and intake including consultation and planning with individuals being referred for services, including learning about needs, goals and dreams
  • Previous supervisory experience, while maintaining the ability to work independently on projects and independent tasks.
  • Proven ability to provide effective leadership within a team environment with respect, professionalism, and cultural awareness
  • Ability to work within the person-led framework
  • Excellent time management, organizational skills along with professional verbal and written communication
  • Proven knowledge and ability in decision-making, problem-solving, conflict resolution, crisis management and intervention
  • Applicants are required to have a valid driver's license and access to a reliable vehicle for work
  • Proficiency in using computer systems, including Microsoft Office and Outlook
  • Experience working with high-risk youth struggling with behavioral issues, mental health, addictions, and exploitation

Responsibilities

  • Completes all forms and plans as needed for the intake and referral process
  • Use Personal Outcome Measures as a tool to determine what quality of life looks like for each person and use this information to inform support plans for new intakes
  • Case management
  • Provides supervision to Case Managers and Homes Supervisors
  • Completing documentation and administration according to agency and government standards
  • Financial responsibilities for the individuals, service area and agency
  • Included in the service areas On-Call rotation
