About The Position

The Youth Services Case Manager implements and supports programs to support youth, and young adults, including YYA who are homeless and at risk of homelessness. This position collaborates with an interdisciplinary team to offer holistic services in both individual and group settings, supporting health, vocational, and self-sufficiency goals; works with community partners to access stable housing and other basic needs assistance, promotes healthy behaviors and positive youth development. Develop service plans with clients, advocates for youth voice, and works as a member of the Homeless Resource Team to support community members securing housing and support services.

Requirements

  • Associate degree in behavioral or health sciences or related field.
  • Bilingual in English/Spanish.
  • Two years of closely related work experience.
  • Knowledge of effective case management practices.
  • Knowledge of government benefits and community resources.
  • Experience in effective social service delivery.
  • Knowledge of addiction and mental health pathologies.
  • Ability to successfully pass a background check in relation to the position applied for.
  • Must have valid driver's license and car insurance.
