About The Position

The Case Manager will adopt a Growth Focused Case Management framework to engage youth and families in achieving stability, educational success, and positive community connections. This innovative approach emphasizes empowerment and utilizes strengths-based strategies, fostering collaborative coordination of services tailored to the unique needs of each participant. By leveraging both internal and external partnerships, the Case Manager will provide comprehensive service coordination and vital information to youth and their families on a range of social and economic issues. This includes legal assistance, social services, medical referrals, educational resources, housing solutions, employment opportunities, and transportation support.

Requirements

  • High School Diploma/GED with 7 years' experience working with a human services organization or Associate degree in human services or related field with three years' experience working with a human services agency or organization in case management or care coordination, required.
  • Minimum 1-2 years experience in case management or youth mentorship within non-profit, educational, or justice-involved settings.
  • Ability to develop and maintain rapport with diverse groups of youth that encourages empowerment, self-sufficiency, and non-dependence.
  • Demonstrated ability to perform programmatic and case management functions.
  • Understanding of the impact of trauma on youth development.
  • Experience facilitating group activities.
  • Must be able to interact effectively with youth and their caregivers of various ages, ethnic, cultural backgrounds, and economic status.
  • Must have knowledge of community, educational and economic resources, and social services throughout Hennepin County.
  • Highly developed organizational skills and comfortable taking initiative.
  • Excellent oral, written and group communication skills.
  • Must have a valid driver's license, reliable transportation and the ability to meet NorthPoint's driving standards. Must be able to drive NorthPoint's passenger van to safely transport youth to and from programming.
  • Must be proficient in MS Word, MS Excel, and computer client tracking databases.

Nice To Haves

  • Bachelor's degree in human services or related degree, preferred.
  • Experience with justice-involved youth preferred.
