Position Summary: The Case Coordinator is assigned responsibility for providing elements of the casework services to individuals and families. The Case Coordinator has the primary responsibility for arranging for the provision of services to the individual and family; managing the implementation of the individual's service plan; monitoring the integration of services delivered by the provider to each individual; case recording and the scheduling of periodic reviews. The overall responsibility for maintenance of the service plan rests with the Case Coordinator. Primary Duties & Essential Functions: Comply with all requirements of 291 licensure. Compliance with applicable laws, rules and regulations to include NYSED Guidelines. Work in conjunction with Vice President of Operations to anticipate openings and discharges and monitor the status of referrals. Assist in preparation of case presentation to treatment team. Upon admission, or within 24 hours to meet or have telephone contact with the individual and family to assist in the transition, complete necessary paperwork and review policy and procedures. Arrange for discharge and aftercare services, attend meetings with individual and family, complete necessary paperwork and provide follow-up as needed. Maintain regular casework contact with individuals and discuss issues, and address concerns. Regular contact with parents/guardians to keep them informed of individual's progress, medical issues, arrange visitation plans, inform of relevant meetings (case conference, service plan reviews, IEP meetings), obtain releases for information, field trips and emergency situations. Provide regular ongoing contact with referral/collateral agencies (DSS, CSE, Office of Children and Family Services) to review individual's progress, inform of visitation plans, inform relevant meetings and emergency situations. Arrange and provide casework related transportation as needed. Arrange school and summer programs and complete school related forms. Arrange and manage services for individual and families. Attend supervision and staff meeting as scheduled, participate in-service training, attend conferences when possible or as requested. Coordinate, monitor and record individual appointments and schedule logistics around appointments in the least disruptive manner. Coordinate information and prepare reports for Court hearings (discharges, extensions). Schedule and record visitation arrangements. Schedule CTP's and Service Plan Reviews. Ensure all documentation is included in the FASP. Timely Progress Notes. Preparation of Permanency Hearing Reports. Assist in maintaining current files on individuals. Complete psychosocial evaluations within 30 days of admission. Notify Health Services personnel, parents/guardians, Clinician(s), school personnel, referral/collateral agency personnel, consultants/specialists immediately of medication changes, medical emergencies/critical incidents, and address jointly with resident staff. Maintain contact with school personnel to review individual's progress and communicate strategies to maintain consistency in the school setting. Attend CSE and school related meetings. Maintain contact with Health Services personnel and request medical services as needed. Maintain frequent contact with Clinician(s) to discuss therapeutic issues and approaches. Attend Treatment team Meetings, case conferences, and chair service plan reviews. Work in conjunction with treatment team members to develop and implement strategies for consistent treatment. Maintain regular contact with Direct Care Supervisor, House Manager to discuss individuals progress and concerns, review unit events, make judgments/decisions regarding individuals, and assist in establishing unit rules and consequences for non-compliance by individuals. Attend weekly Clinical Consult Meeting with Clinician and Residential House Manager. Provide and receive feedback on programs, individual's daily functioning and behaviors and difficulties which need to be addressed. Work in conjunction with Clinical/Treatment Team to arrange for needed Consultant/Specialist evaluations and medication reviews, complete medication review form. Attend and represent individual in Family and Criminal Court with collateral agencies. Provision of Life Skills Group and Psycho Educational Groups on Sanctuary.
