About The Position

Holon Health is rebuilding the healthcare system for justice-involved individuals living with Substance Use Disorder (SUD), focusing on whole-person care that meets clients where they are and builds pathways to long-term recovery. They are seeking an experienced Youth Nurse Practitioner to join their forward-thinking team and break down barriers to behavioral healthcare.

Requirements

• Active and unrestricted NP license in Tennessee
• Active DEA license (and CDS where applicable)
• Several years of experience in behavioral health, addiction medicine, or psychiatry
• Experienced in working with youth populations and their caregivers, specifically those in the 12-17 age range
• Experienced in addiction medicine or psychiatry, with a commitment to person-centered care
• A strong collaborator who thrives in integrated and team-based care models
• Organized, flexible, and capable of navigating complex systems
• Clear and confident in virtual communication with both clients and team members
• Strong documentation skills and proficiency in EHR platforms

Nice To Haves

• Multiple and additional state licenses are a plus
• Board certification in specialty preferred

Responsibilities

• Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
• Diagnose health conditions and create individualized treatment plans
• Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
• Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
• Serve as a clinical consultant on complex or high-need cases
• Educate clients and families on treatment plans, medications, and recovery support strategies
• Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
• Participate in team meetings, professional development, and continuous training
