The Navigator is responsible for providing comprehensive case management, care coordination, outreach, and navigation services to transition-age youth and young adults (ages 16–26). The Youth Navigator serves as a trusted partner, helping participants identify strengths, overcome barriers, and access coordinated services that promote health, education, employment, housing stability, behavioral health, and overall well-being. The Youth Navigator will build collaborative relationships with community providers, educational institutions, workforce partners, healthcare organizations, and social service agencies to ensure participants receive timely, coordinated, and person-centered care. This position utilizes trauma-informed, culturally responsive, and youth-driven practices to empower young adults toward self-sufficiency and long-term success.
