As a Youth Counselor, you will play a vital role in providing direct counseling and case management services to youth between the ages of 2 – 17 and their families. This community-based position emphasizes holistic support, including connecting clients with natural supports and local resources. You will deliver mental health and crisis intervention services across diverse settings such as homes, schools, and community locations. Immediate Stabilization Services (ISS) is a community-based program that supports young people, their families, and/or resource parents in times of crisis and high need to reduce the likelihood of placement disruption. Prevention and stabilization services are available 7 days a week. ISS works with young people who are dependents of Santa Clara County or at risk of child welfare involvement, including those served through hospital diversion. A family enrolled in ISS will work with an assigned clinician, a facilitator, and a team of counselors to provide individualized interventions. The ISS team collaborates with youth and caregivers to create trauma informed care plans designed to increase safety and prevent placement disruption, so that youth and caregivers feel empowered and supported while in the program.
