About The Position

This position is responsible to act as a liaison between Four Oaks programming and the Cedar Rapids School District daily school operations on the Four Oaks campus. This position works at school during the school year on in-school days, and on the Four Oaks program unit during school breaks or non-school days.

Requirements

  • At least 21 years of age
  • Bachelor's Degree plus one (1) year of relevant experience with At Risk populations, and/or school support services OR meets Youth Counselor requirements plus added relevant experience.

Responsibilities

  • Walks Four Oaks youth to/from program units to the school classrooms as needed.
  • Provides direct supervision of students throughout school day activities.
  • Completes light custodial work as needed during the school day.
  • Assists the School Facilitator as needed.
  • Distributes medications, attends to first aid needs, and takes youth to medical/dental appointments as scheduled.
  • Helps in processing behavioral incidents that occur at school.
  • Participates in the school team meetings as well as STOP clinical meetings to effectively communicate needs between the school and Four Oaks.
  • Establishes a therapeutic relationship between Four Oaks & CRSD school teams, fostering mutual trust and respect.
  • Assess student health and wellness to ensure appropriate school attendance.
  • Participates in inter-agency team meetings, trainings and resources as needed to achieve student and family outcomes.
  • Mediates conflict and problem-solves real and perceived barriers for students.
  • Works in the direct care ratio at the onsite school as well as the Four Oaks units at Bertram as needed and appropriate. (School year on the school site; school breaks on the treatment units).
  • Maintains a school environment that is up to physical safety standards of cleanliness as needed during the school day (ex: clean up spills or accidents, clean restrooms).
  • Establish and maintain regular communication cycles between Four Oaks and the school team, including attending the school team's weekly meeting and the Four Oaks' weekly clinicals).
  • Knows the population and is aware of individual treatment plans, safety concerns, and assists teams at school and Four Oaks to understand and implement these plans.
  • Assists with the implementation of groups to promote success.
  • Assists teachers with developing transition plans for students (appointments, medication needs during the school day, planned breaks at school or the unit, etc.)
  • Attends quarterly meetings as requested with Four Oaks Education Director and CRCSD leadership for ongoing program quality assessments and needs.
  • Performs all other duties as assigned.
