About The Position

Position Summary: This position is responsible for managing the youth Coordinated Entry (CE) priority list to support an equitable system for housing referrals for youth experiencing homelessness in the Twin Cities. The Youth CE Priority List Manager collaborates with Ramsey County Continuum of Care (CoC), youth-serving agencies, and priority list managers for domestic violence, singles, and families to coordinate referrals and ensure system alignment. This role may also work directly with unhoused youth to complete coordinated entry assessments and connect them to resources. Services are provided using a client-centered, trauma-informed, and anti-racist approach. This position is primarily remote, with required on-site days and community-based work, including occasional travel.

Requirements

  • Minimum of two (2) years of experience in program development, project management, housing advocacy, or outreach within nonprofit or government systems.
  • Experience working with youth who have experienced trauma.
  • Strong organizational skills with the ability to manage multiple priorities and maintain attention to detail.
  • Effective communication and interpersonal skills.
  • Ability to work independently and collaboratively in a team environment.
  • Strong problem-solving skills and ability to develop creative solutions.
  • Demonstrated cultural competency and commitment to anti-racism and trauma-informed practices.
  • Ability to work effectively across diverse populations and experiences.
  • Commitment to creating inclusive and safe environments, including for LGBTQ+ individuals.
  • Ability to collect, manage, and maintain confidential data.
  • Ability to facilitate meetings, trainings, and consultations.
  • Strong written and verbal communication skills.
  • Ability to engage in reflective practices related to power, privilege, and equity.
  • Proficient in data systems and Microsoft Office applications; HMIS experience preferred.
  • Valid driver’s license and current auto insurance required.
  • Ability to pass a background check.

Nice To Haves

  • Experience working with Coordinated Entry systems (CE/CEYS) and CAHS preferred.
  • Familiarity with HMIS and data reporting preferred.
  • Existing relationships within Ramsey County CoC preferred.

Responsibilities

  • Maintain and manage the Ramsey CoC youth priority list (ages 16–24).
  • Collaborate with priority list managers for singles, families, and domestic violence programs to ensure alignment.
  • Track referrals to housing providers and monitor outcomes.
  • Ensure housing providers update the priority list and remove individuals once housed.
  • Identify and track provider compliance and report concerns to the CoC.
  • Assist youth with coordinated entry assessments and provide referral information.
  • Connect youth to CE Youth Navigators for assistance with documentation and housing readiness.
  • Respond to client calls and emails and provide appropriate support and referrals.
  • Run and analyze HMIS reports and share findings with the CoC Coordinator and stakeholders.
  • Maintain accurate client records and ensure compliance with confidentiality requirements, including VOCA standards.
  • Track and report vacancy data from housing providers.
  • Develop tools and procedures to improve coordinated entry system efficiency.
  • Facilitate youth case consultations in alignment with HUD prioritization guidance.
  • Participate in CoC meetings, workgroups, and priority list manager meetings.
  • Provide training and consultation to housing providers and assessing agencies.
  • Maintain and update CE-related training materials and resources.
  • Support development of strategies to incorporate youth feedback into coordinated entry processes.
  • Promote and support mobile and community-based assessment sites.
