About The Position

Zepf Center is seeking a full-time Youth Community Based Case Manager to provide community support and coordination of services for youth experiencing serious mental illness and/or addiction challenges. The Case Manager will support clients in achieving their short and long-term goals and accessing community resources using a person-centered approach, respecting cultural preferences and client rights. This role involves developing comprehensive care coordination and treatment plans, advocating for clients, utilizing evidence-based assessments, providing referrals, coordinating crisis intervention, and supporting clients' overall health and wellness. Services are delivered in the community, client homes, and agency locations. The position offers opportunities for quarterly bonuses based on client engagement.

Requirements

  • HS Diploma and/or equivalent with three years of experience OR Bachelors or Masters degree in a related field
  • Valid Driver’s License
  • QHMS certification required (at minimum) throughout employment
  • Must be able to safely drive in all-weather conditions in support of client needs

Responsibilities

  • Assess client strengths and needs with the client and applicable team members, conduct comprehensive care coordination, and develop treatment plans.
  • Monitor and evaluate client progress toward goal attainment and review/modify treatment plans as needed.
  • Maintain contact with the client as indicated in the treatment plan or as needed to ensure stability.
  • Communicate with and provide support to clients in achieving goals and outcomes.
  • Provide active outreach, including crisis intervention and management, and acquisition of appropriate community supports.
  • Link and/or refer clients to other services, programs, and agencies as needed.
  • Assist in assuring clients obtain and retain applicable entitlements and follow up on same.
  • Serve as an advocate for clients.
  • With appropriate releases, communicate with family members and significant others as indicated.
  • Confer with supervisor and other team members regarding clients and, as appropriate, other agencies with emphasis on provision of most appropriate, effective, and cost-efficient services.
  • May provide face-to-face contact with clients who are hospitalized to facilitate discharge planning and coordination of services.
  • Work with hospital and agency medical staff regarding discharge planning.
  • Assist clients in tracking and keeping appointments.
  • Assist clients with developing skills that lead to independence.
  • Provide back up for other staff.
  • Complete required documentation in the electronic health record in accordance with timelines and standards required by the agency (within 48 hours of interaction, regulatory bodies, and payors).
  • Demonstrate excellent written and oral communication skills.
  • Carry out policies and procedures of the agency.
  • May participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings, and perform other related duties as requested.
  • Complete required training including CPR, CPI, new hire, Sanctuary and annual boosters in addition to any required trainings assigned during employment.
  • Safely drive in all-weather conditions in support of client needs.
  • Perform other duties as assigned.
