Provide individualized support and direct assistance to program participants in all areas related to gaining and/or retaining employment and career advancement. This role involves counseling, motivating, and guiding participants through customized career pathways, training opportunities, and barrier mitigation to achieve successful placement outcomes. A key aspect is building a professional one-on-one working relationship to aid in eliminating challenges to obtaining and retaining employment, which includes assessment, individualized attention, and coordination of referrals to appropriate community or social service agencies. The position requires maintaining and documenting all participant interactions, contacts, and activities in case notes and files, ensuring regularly scheduled contacts are made as required. Accurate and timely submission of required program data for real-time reporting is essential. The role also involves accurately completing required documentation for enrollment, developing and maintaining customized service plans aligned with participant employment and training goals, and following up with partner agencies to ensure successful engagement in prescribed support services. Outreach to support participant engagement through off-site appointments, satellite site locations, and community visits is also a responsibility. Consistent and comprehensive case management throughout program engagement to monitor participant activities and maintain organized tracking of clients is crucial. Detailed documentation of all contacts in the state’s system of record per contact requirements, coordination of participant retention services, and accommodation of participant work schedules are also key functions. Finally, providing status updates and developing goal planning sessions with appropriate program staff are part of the role.
