Within a team-based model of care, the Youth Care Worker provides treatment services to youth with serious mental health and substance use issues. They collaborate with the interdisciplinary team to assist in managing client behavior and contribute to the development of behavioral care plans. The role involves developing, implementing, and adjusting treatment plans, and assisting clients in achieving their individualized treatment goals. Care is provided through trauma-informed practice, harm reduction, and a recovery-oriented approach. The position includes co-facilitating psycho-educational groups, providing education based on theoretical models, and participating in team care planning for youth mental health, substance use, and concurrent disorders prevention/early intervention strategies. This includes participating in case conferences and meetings with the interdisciplinary team for the identification and management of youth mental health and substance use issues.
