About The Position

The Wraparound Facilitator (YES Waiver) is responsible for assessing, planning, coordinating, monitoring, and linking patients with mental illness to service providers. The Wraparound Facilitator acts as a broker of services and assures that services are meeting the patient's needs. The Wraparound Facilitator (YES Waiver) is responsible for the development of the wraparound plan, individual plan of care, assessments and required documentation for the assigned caseload. The position may also provide rehab services for patients including crisis management and intervention. The Wraparound Facilitator (YES Waiver) utilizes the National Wraparound Initiative model of intensive case management, and this position will/must be trained in this initiative as a Wraparound Facilitator. Services must be provided according to all applicable standards and established performance and compliance benchmarks. This position will work closely with comprehensive service providers to ensure that individuals are receiving appropriate, quality services based on individual need as determined by the treatment team and the family. The position must be able to effectively lead a wraparound team and have the ability to network and work collaboratively with others. The Wraparound Facilitator (YES Waiver) is responsible for documenting and data entering services in a proficient and accurate manner and according to required standards. The Wraparound Facilitator (YES Waiver) as a member of a multi-disciplinary treatment team. In addition, the Wraparound Facilitator (YES Waiver) performs other duties as assigned by the supervisor.

Requirements

  • Bachelor's or advanced degree from an accredited college or university
  • Texas driver’s license in good standing as defined by Texana Center’s procedure
  • Must be able to perform SAMA and CPR.
  • Must be able to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
  • Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
  • Must be able to perform these functions with or without reasonable accommodations.

Nice To Haves

  • 1 year of work experience providing direct services within a Behavioral Health or Social Service
  • Trained in National Wraparound Initiative model of intensive case management
  • Experience with the National Wraparound Initiative model of intensive case management
  • Spanish speaking
