About The Position

As part of the person-centered care model, this role will develop and coordinate agency-wide programs for the prevention and management of skin breakdown. The specialist provides expert consultation and/or hands-on care for individual patients with acute or chronic wounds, establishing and coordinating multi-disciplinary, comprehensive management plans. The role also involves coordinating care for patient populations, identifying trends, and developing standardized prevention and treatment protocols, which are then refined based on outcomes. Key responsibilities include developing and implementing educational programs for patients, staff, and other healthcare providers, as well as coordinating quality improvement initiatives to optimize skin and wound outcomes. The Wound Program Specialist acts as an advocate for patients with skin and wound care needs.

Requirements

  • Bachelor’s Degree in nursing required
  • WOCN certification required
  • Minimum of two years nursing experience in clinical specialty area required
  • RN required
  • Obtain Wound/Ostomy certification within 6 months of hire required
  • Strong written and verbal communication skills
  • Ability to effectively communicate at all levels within the organization and share knowledge, ideas and information
  • Demonstrated success in management, planning and leadership ability for meeting patient outcomes
  • Able to understand problem situations, solve problems and independently assess a wide variety of tasks in order to effectively take action to identify solutions to benefit the client/agency
  • Knowledge of relevant industry standards for wound care treatments
  • Ability to balance financial, quality, people and customer expectation appropriate to business situation
  • Intermediate to advanced MS Outlook, Word, Excel and PowerPoint, and Microsoft Project

Responsibilities

  • Develop/coordinate agency wide programs for prevention and management of skin breakdown
  • Provide expert consultation and/or hands-on care for individual patients with acute or chronic wounds
  • Establish/coordinate multi-disciplinary, comprehensive management plans
  • Coordinate care for populations of patients, identifying trends, and developing standardized prevention and treatment protocols and refining protocols based on outcomes
  • Develop and implement educational programs for patients, staff and other health care providers
  • Coordinate quality improvement initiatives to optimize skin and wound outcomes
  • Advocate for the patient with skin and wound care needs
  • Manage regional wound care program metrics ensuring consistent delivery of quality care and communications
  • Collaborate and explore opportunities for cost effectiveness for wound treatments in relationship to patient needs
  • Review Quality reporting data for deficiencies, areas for improvement, barriers to safety
  • Seek consultation with other regional WOCN staff to identify and plan interventions designed to solve complex problems involving client care
  • Develop an interdisciplinary approach to client care between Wound program, HHCAH clinical staff and community partners
  • Act as consultant to help coordinate all client care services with community partners and agencies as required for the patient to achieve outcomes
  • Inform the physician/wound clinics of changes in client needs, wounds that require change to the plan of care for wound care treatments and disease management
  • Follow trends in specialty area of wounds for home care industry; make recommendations of treatment options for incorporation into the program
  • Participate in reviewing regional wound program performance metrics to ensure program is achieving desired clinical and operational performance measures with Regional Director and Manger of Specialty Services
  • Participate in training, coaching of staff and onboarding to the Wound program
  • Participate and facilitate professional development needs and competency for wound program
  • Actively participate in the performance improvement process known as H3W
  • Participate in clinical education area to provide programs aimed to improve clinical competencies of direct service staff
  • Research new information related to wound specialty and communicate to team
  • Perform other duties as assigned
