About The Position

Jefferson Health at Home by BAYADA is recruiting a compassionate Wound, Ostomy, and Continence Nurse to provide home-based nursing care to patients throughout King of Prussia, Pennsylvania. The nurse supports patients from hospital discharge through safe living at home, helping them maintain comfort, independence, and dignity. The role includes direct one-to-one skilled nursing care, wound care consultations, oversight, assessments, patient and family education, and monitoring patients’ conditions. The position is offered full-time, part-time, or per diem and follows a Monday-through-Friday schedule with no weekends or holidays. Scheduling is primarily during daytime hours.

Requirements

• Active, current Pennsylvania license to practice as a professional registered nurse (RN).
• Wound, Ostomy and Continence Nurse (WOCN) certification.
• Ability to travel to cases as assigned.
• Calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
• Ability to deliver patient care with compassion, excellence, and reliability.

Nice To Haves

• Prior home care experience is strongly preferred, but not required.

Responsibilities

• Guide clients through their care needs from hospital discharge to safe home living with comfort, independence, and dignity.
• Provide one-to-one skilled nursing, interventions, and supportive care according to each patient's care plan, including medication administration, wound care, treatments, and comfort care.
• Conduct wound care consultations and provide oversight.
• Assess clients, educate patients and/or their families, and monitor patients’ conditions.
• Travel to assigned cases.
