About The Position
Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are known for our small-town atmosphere, strong community connections, and commitment to providing compassionate, high-quality care. We are seeking an experienced and dedicated Wound Care LPN to join our clinical team. This is an excellent opportunity to specialize in wound care while making a meaningful impact on residents’ health and recovery.
Requirements
- Active, unencunbered LPN license in South Carolina
- Graduate of an accredited Practical Nursing program
- Current CPR certification required
- Ability to work 12-hour shifts
Nice To Haves
- Experience with PointClickCare (PCC) preferred
- Prior wound care experience strongly preferred
Responsibilities
- Provide direct wound care and skin treatments in accordance with physician orders
- Assess, monitor, and document wound progress and treatment effectiveness
- Assist in developing and modifying individualized care plans
- Collaborate with the interdisciplinary team to ensure optimal outcomes
- Educate staff on proper wound care techniques and prevention strategies
- Maintain accurate and compliant clinical documentation
- Ensure adherence to all regulatory, safety, and infection control standards
