About The Position

Home Care Agency is seeking caring, compassionate, highly qualified, dependable, and experienced Licensed Practical Nurses (LPNs) with G-Tube feeding and wound care experience. The role involves providing exceptional one-on-one direct care to patients in their homes in locations such as Smithfield, Garner, and Apex.

Requirements

• Wound Care and G-Tube Feeding Management experience required
• Must possess a current, unencumbered, active state license to practice as a Licensed Practical Nurse (LPN)
• Current BLS Certification
• Excellent verbal and written communication skills
• Must be qualified, compassionate, and dedicated to a job well done
• Reliable Transportation

Nice To Haves

• Prior home care experience preferred but not required

Responsibilities

• Experience with Wound Care and G-Tube Feeding Management
• Provide one-on-one direct care in the home
• Investigates and resolves patient/family member concerns regarding patient care
• Ensures the highest quality of care is provided to patients by reviewing documentation
• Observe, assess, documents and reports patient response to treatment
• Medication administration and management
• Experience with proper documentation of skilled nursing care in home health
• Understands Dr’s order to complete in-home skilled nursing care
• Observe, assess, documents and reports patient response to in-home treatment
