Requirements

• Must possess a current, unencumbered, active state license to practice as an LPN/RN.
• Prior home care experience preferred
• Current BLS Certification
• Excellent verbal and written communication skills
• Must be qualified, compassionate, and dedicated to a job well done
• Reliable Transportation

Responsibilities

• Provide one-on-one direct nursing care in the home
• Review, initiate and implement care plan per client’s needs
• Medication management and administration
• Assist with ambulating, hygiene, toileting, dressing
• Ensure client has fluids regularly
• Manage oxygen machine
• Follow Dr’s order for patients care
• Experience with proper documentation of client care
• Observe, assess, documents and reports patient response to treatment to DON
• Investigates and resolves patient/family member concerns regarding patient care
• Ensures the highest quality of care is provided to patients by reviewing documentations
