Requirements
- Must possess a current, unencumbered, active state license to practice as an LPN/RN
- Prior home care experience preferred
- Current BLS Certification
- Excellent verbal and written communication skills
- Must be qualified, compassionate, and dedicated to a job well done
- Reliable Transportation
Responsibilities
- Provide one-on-one direct nursing care in the home
- Review, initiate and implement care plan per client’s needs
- Medication management and administration
- Assist with ambulating, hygiene, toileting, dressing
- Ensure client has fluids regularly
- Manage oxygen machine
- Follow Dr’s order for patients care
- Experience with proper documentation of client care
- Observe, assess, documents and reports patient response to treatment to DON
- Investigates and resolves patient/family member concerns regarding patient care
- Ensures the highest quality of care is provided to patients by reviewing documentations
