Provides coordination of care to patients with select disorders of the GI, GU, and Integumentary systems including and not limited to abdominal stomas, wounds, fistulas, drains, tubes, pressure ulcers and incontinence. The Wound Clinic Coordinator provides complex clinical nursing care, functions as a resource within the hospital and community, a coordinator of care and educator to patients and staff. Provides leadership to staff in the form of coaching, mentoring, training, professional development, process evaluation and improvement. This involves structured visits, in-service presentations, and participation in multidisciplinary meetings when appropriate. Clinical Nursing: Provides first-in-class patient care to a group of patients, provides specific assessments according to protocols and procedures, patient/caregiver education related to wound, ostomy and continence care management. Provides strong collaboration and direction to nursing staff and actively working with physicians, nurses and clinical staff to ensure exceptional care.
