Requirements

• Compassionate, skilled, and dedicated RN/LPN committed to excellence in nursing care.

Responsibilities

• Assess residents’ physical, emotional, and psychosocial needs and develop individualized care plans with the interdisciplinary team.
• Administer prescribed medications and treatments, monitor for adverse reactions, and educate residents about medication management and side effects.
• Monitor vital signs, symptoms, and changes in residents’ conditions, and initiate appropriate interventions.
• Provide direct nursing care, including wound care, catheter care, and assistance with activities of daily living.
• Supervise and delegate tasks to licensed practical nurses (LPNs) and certified nursing assistants (CNAs).
• Document resident assessments, care plans, interventions, and outcomes accurately and promptly in accordance with regulatory standards and facility policies.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
