About The Position

Cleveland Health Care Center is seeking a compassionate and clinically skilled Wound Care / Treatment Nurse (LVN or RN) to join our nursing team. This is a specialized nursing role focused on wound management, skin integrity, pressure injury prevention, and treatment oversight within our skilled nursing facility. The ideal candidate is detail-oriented, proactive, and passionate about improving resident outcomes through evidence-based wound care practices. Requirements

  • Current Texas LVN or RN license in good standing
  • Current CPR certification
  • Strong assessment and documentation skills
  • Excellent communication and organizational abilities

Nice To Haves

  • Skilled Nursing Facility (SNF) or Long-Term Care experience
  • Previous wound care or treatment nurse experience
  • Wound Care Certification (WCC, WOCN, or equivalent)
  • Knowledge of CMS regulations
  • Knowledge of infection prevention practices
  • Knowledge of pressure injury prevention
  • Knowledge of skin integrity management

Responsibilities

  • Conduct comprehensive skin assessments upon admission, readmission, and as needed
  • Assess, stage, measure, photograph, and document wounds accurately
  • Develop and update individualized wound care plans
  • Perform treatments and dressing changes in accordance with physician orders
  • Monitor wound healing progress and communicate changes to providers
  • Lead facility skin rounds and pressure injury prevention initiatives
  • Collaborate with dietary and therapy teams to support wound healing
  • Maintain wound tracking logs and treatment documentation
  • Monitor compliance with facility protocols and regulatory requirements
  • Participate in QAPI and quality improvement initiatives
  • Assist with survey preparation and regulatory compliance efforts
  • Investigate and follow up on skin integrity concerns and treatment outcomes
  • Provide ongoing education and guidance to nursing staff on wound assessment and staging, pressure injury prevention, infection prevention, treatment protocols, and documentation requirements
  • Serve as a clinical resource and mentor for wound care best practices
  • Assist with nursing assessments and resident care as needed
  • Participate in the nursing leadership on-call rotation every third weekend
  • Support the Director of Nursing and interdisciplinary team with clinical initiatives
