As part of the person centered care model, this role will participate in the daily management and delivery of wound care for the Wound program. This role will ensure the delivery of outcome based wound care, cost effective care by utilizing metric-driven decisions. This role will collaborate with Wound team members to achieve exceptional outcomes. The Wound team RN performs wound care and oversight of clinical staff in the daily delivery of services and maintenance of clinical care standards for clients receiving the specialized wound care treatments. Develops, directs, and manages the daily operations and delivery of wound care to patients, and participates in the development, maintenance and implementation of program goals and objectives. Manages a wound care case load and LPN staff ensuring consistent delivery of quality care and communications; collaborates and explores opportunities for cost effectiveness for wound treatments in relationship to patient needs Seeks consultation with WOCN staff to identify and plan interventions designed to solve complex problems involving client care. Develops and interdisciplinary approach to client care with Wound program/Agency staff. The Wound Program RN will direct and coordinate all client care services for both Wound care and medical/surgical services required by the patient to achieve outcomes. Informs the physician/wound clinics of changes in client needs, wounds that require change to the plan of care for wound care treatments and disease management Determines staffing needs and coordinates patient scheduling to meet service demands. Accountable for team performance in achieving desired clinical and operational performance measures. Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program. Participates in training, coaching of staff and onboarding to the Wound program Participates and facilitates professional development needs and competency for wound program. In addition, the position involves other duties such as: 1. Will actively participate in the performance improvement process known as H3W. 2. Participates in clinical education area to provide programs aimed to improve clinical competencies of direct service staff. 3. Researches new information related to wound specialty and communicates to team. 4. Performs other duties as assigned.

Requirements

  • Associates degree, diploma in nursing.
  • Minimum of two years nursing experience in clinical specialty area required.
  • RN License required.
  • Obtain Wound/Ostomy certification within 6 months of hire required.
  • Ability to effectively communicate at all levels within the organization and share knowledge, ideas and information.
  • Demonstrated success in management, planning and leadership ability for meeting patient outcomes.
  • Able to understand problem situations, solve problems and independently assess a wide variety of tasks in order to effectively take action to identify solutions to benefit the client/agency.
  • Knowledge of relevant industry standards for wound care treatments.
  • Ability to balance financial, quality, people and customer expectation appropriate to business situation.
  • Intermediate to advanced MS Outlook, Word, Excel and PowerPoint, and Microsoft Project.

Nice To Haves

  • Wound/Ostomy specialty certification preferred.
  • Bachelor’s Degree in nursing preferred.
  • Minimum two years in homecare preferred.
