About The Position

OakLeaf Clinics - Wound Care has an exciting opportunity to join our expanding team as a full-time, 0.8 FTE Registered Nurse! OakLeaf Clinics is dedicated to providing our patients with compassion, trust, and a lifetime of individualized care. Our healthcare team consists of physicians, nurse practitioners, physician assistants, dietitians, nurses, respiratory therapists, and medical assistants working in concert with laboratory/imaging services to offer individualized care to the Chippewa Valley.

Requirements

  • Graduate of an accredited school of nursing
  • Current State of Wisconsin Registered Nurse License or Nurse Compact License
  • Basic Life Support (BLS) certification
  • Medical terminology knowledge and understanding of patient care notes
  • Ability to demonstrate effective written and verbal communication skills to complete documentation for medical record purposes
  • Excellent interpersonal and customer service skills
  • Detail-oriented and accurate
  • Passionate about providing patient-focused care
  • Ability to multitask with attention to detail

Nice To Haves

  • Prior Epic experience preferred

Responsibilities

  • Assess the patient's needs to make appropriate and independent/collaborative decisions for selecting a specific triage plan
  • Use departmental evidence-based protocols to triage patients
  • Instruct patients on their healthcare
  • Document all interactions with patients and providers when medical direction is given
  • Coordinate services with other departments for follow-up based on the patient's needs
  • Communicate problems, concerns, and suggestions to lead nurses or the clinic manager
  • Maintain up-to-date knowledge and skills in professional and clinical areas
  • Provide professional nursing care under supervision or within the scope of licensure for clinic patients
  • Facilitate/perform the gathering of patient information and document accordingly
  • Schedule follow-up appointments
  • Instruct patients and families by phone as directed within the scope of licensure or certification
  • Screen patients for appropriate information
  • Maintain a timely flow of patients and complete forms as needed
  • Assist with office procedures as directed
  • Participate in professional development activities and maintain professional affiliations
  • Maintain patient confidentiality
