Requirements

  • Graduation from an accredited nursing school – RN or LPN with active licensure to practice in the state of employment.
  • Current Wound Care Certification required, with CPR certification.
  • A minimum of 1 year of wound care experience (preferably 2 years) in a healthcare setting.
  • Must be compassionate, detail-oriented, and dedicated to providing the best care for residents.
  • Ability to collaborate with physicians, nursing staff, and interdisciplinary teams effectively.
  • Familiarity with corporate and external regulatory guidelines related to wound care and healthcare practices.

Responsibilities

  • Serve as the primary contact between the Wound Specialist Physician and the nursing team, ensuring seamless communication and care coordination.
  • Actively participate in the creation and achievement of Wound Care Program objectives, ensuring alignment with regulatory guidelines and best practices.
  • Provide staff development by delivering educational programming to promote wound care knowledge and best practices within the nursing team.
  • Assist in the implementation and compliance of wound care policies, procedures, and standards, ensuring the facility meets all regulatory requirements.
  • Work closely with the Interdisciplinary Team to address all aspects of care that contribute to effective wound healing.
  • Assess, treat, and monitor residents' wounds as ordered by the physician, ensuring appropriate care, follow-up, and resident comfort.
  • Assist in creating personalized treatment plans for each resident, promoting effective healing and improved outcomes.
  • Maintain ongoing communication with physicians to provide updates on resident care and ensure a comprehensive approach to wound management.
  • Assist in facilitating regulatory compliance during surveys and inspections as directed by the Director of Clinical Services.
  • Continuously look for opportunities to improve systems and processes that enhance the quality and effectiveness of wound care and overall resident outcomes.
