About The Position

The Wound Care Nurse coordinates treatment to provide comprehensive wound assessments and plan of care to provide patient & family education regarding prevention of pressure ulcers and support services, dressing selection and adjunctive therapy. The Wound Care Nurse will collaborate with clinical staff, physicians and other members of the treatment team to provide the patient with the highest quality and most appropriate wound care. Acts as a resource for the treatment team and clinical staff.

Requirements

  • Must be a graduate of an accredited school of nursing (RN or LVN) with a current license in the State of Texas.
  • The nurse should preferably have a certification of wound care through an accredited program, with at least 1 years-experience.
  • Must have knowledge of treatment protocols and how to assess skin and stage pressure ulcers.
  • Must have experience working with the geriatric population.
  • Good organizational and communication skills
  • Excellent assessment skills
  • Must willingly demonstrate the ability to work in a team-oriented environment
  • Must demonstrate initiative – be a self-starter with job duties assigned
  • Good computer skills
  • Must possess critical thinking skills and be able to make independent decisions
  • Must be able to meet the physical requirements of positioning patients when providing wound care and to ensure interventions are put into place to provide the appropriate care.
  • Must be able to assess all treatments for changes and document these changes as they occur and obtain orders for treatment changes as appropriate.
  • Must have normal dexterity, visual accuracy, color vision, the ability to speak, read, write, and understand English to comprehend instructions.

Responsibilities

  • Coordinates treatment to provide comprehensive wound assessments
  • Plan of care to provide patient & family education regarding prevention of pressure ulcers and support services, dressing selection and adjunctive therapy
  • Collaborate with clinical staff, physicians and other members of the treatment team to provide the patient with the highest quality and most appropriate wound care
  • Acts as a resource for the treatment team and clinical staff
  • Assess skin and stage pressure ulcers
  • Complete all treatments ordered and facility skin assessments for patients
  • Document all treatments in the clinical record, weekly updates
  • Complete skin reports to ensure clinical management team & MDS are familiar with treatment and condition of the wounds
  • Maintain the treatment cart
  • Assist with and manage treatment orders
  • Inform managers and DON/ADON of any changes in condition or treatments
  • Practice good infection control procedures
  • Maintain clinical records to always be complete and ready for review
  • Work with the clinical team for the weekly wound rounds on Wednesdays
  • Educate staff as needed to follow wound care protocols and how to position residents to avoid pressure
  • Maintain professionalism in all circumstances
