About The Position
Caring Place Healthcare Group is seeking a dedicated Wound Care Nurse to lead and support our wound prevention and treatment programs. This role involves assessing, treating, and monitoring residents with acute and chronic wounds, developing individualized care plans, and collaborating with interdisciplinary teams. The position also includes educating nursing staff, monitoring compliance, tracking data, and participating in quality improvement initiatives.
Requirements
- Graduate of an accredited School of Nursing
- Current LPN or RN license in the State of Ohio
- Current CPR certification
- Minimum of 1 year of experience in wound care required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively with a team
- Highly organized with strong time-management skills
- Compassionate, patient-focused approach to care
Nice To Haves
- At least 3 years of experience in healthcare or long-term care preferred
Responsibilities
- Assess, treat, and monitor residents with acute and chronic wounds
- Develop and implement individualized wound care plans and prevention strategies
- Conduct routine rounds to evaluate risk factors and ensure appropriate interventions
- Collaborate with physicians and interdisciplinary teams to coordinate care and treatment plans
- Educate and guide nursing staff on wound care protocols, prevention techniques, and best practices
- Monitor and ensure compliance with all wound care plans and regulatory requirements
- Track and report wound care data, including pressure ulcers and treatment outcomes
- Evaluate new admissions and existing residents for pressure ulcer risk and implement preventative measures
- Ensure proper supply levels of wound care materials and equipment
- Participate in quality improvement initiatives and support regulatory surveys
- Provide education and emotional support to residents and families
