About The Position

We offer Skilled Nursing and Rehabilitation to residents and if you are experienced, motivated, passionate and ready to call The Rivers home, we would love to hear from you! The wound care nurse will provide each resident with routine daily nursing care including but not limited to wound healing, ostomies, and skincare regimens.
Requirements
- Current state license to practice as a LPN or Registered Nurse (RN)
Nice To Haves
- Wound care certification preferred
- One (1) year experience in a long-term care setting preferred
Responsibilities
- Follow all established skin management protocols
- Ability to stage and measure wounds and document per protocol, including documentation on skin conditions other than wounds
- Make weekly rounds with wound care physician
- Ensure care plan is followed
- Work with nursing team to educate CNAs as to proper positioning, training, and skin care
