About The Position

MDB Health Services is seeking an experienced Nurse Practitioner to provide exceptional foot and wound care to long-term care residents. This is a full-time position where you will travel to designated nursing homes in your area to conduct thorough wound assessments, develop personalized care plans, and perform necessary procedures. You will also play a key role in educating patients and facility staff, communicating with healthcare professionals, and maintaining accurate patient records. The goal is to ensure seamless care delivery, optimize patient outcomes, and manage pain related to wounds for patient comfort.

Requirements

• Master’s or Doctoral degree in Nursing from an accredited program
• Current Nurse Practitioner license in Louisiana
• Minimum of 2 years of experience in wound care and/or foot care
• Strong clinical skills in assessment and treatment of wounds and foot conditions
• Independent, organized, and motivated to provide exceptional patient care
• Excellent communication and interpersonal skills

Nice To Haves

• Certification in Wound, Ostomy, and Continence (WOCN) is preferred but not required

Responsibilities

• Provide foot care and/or wound care to residents in long-term care facilities
• Conduct thorough wound assessments and develop personalized care plans for each patient
• Perform wound debridement, dressing changes, and other wound care procedures as needed
• Continuously monitor and assess patients’ progress in wound healing and foot health
• Educate patients and facility staff on effective wound care techniques, prevention strategies, and foot care best practices
• Travel to designated nursing homes in your area
• Communicate regularly with key facility staff, physicians, and healthcare professionals to ensure seamless care delivery and optimize patient outcomes
• Maintain accurate and up-to-date patient records and documentation
• Manage pain related to wounds to ensure patient comfort
