About The Position

The Women's Imaging Patient Navigator will support the Nurse Navigator program, High Risk Screening Program and the Screening Mammography program. This position will provide assistance with scheduling biopsies, coordinating services, facilitate and confirm patient appointments, including those made with labs, diagnostic areas, and specialty physicians, manage referral and patient inbound and outbound messages, and collate pertinent statistics and documents needed for the navigation program. With the general guidance of the Nurse Navigator performs with initiative, ingenuity and independent judgement in coordinating administrative aspect of breast health navigating.

Requirements

High school diploma or GED required
Strong communication and organizational skills
Ability to multitask and work collaboratively in a team environment

Nice To Haves

Prior experience working in a hospital, ambulatory, or medical office setting preferred
Basic clinical and computer skills preferred

Responsibilities

Work with health care professionals and support staff, including those from other disciplines, to provide patient-focused care.
Continuous monitoring and communication of results, within specified timeframe, to patients as required for screening mammography program.
Collate all needed documentation for referring providers, specialist providers, and further imaging procedures ie: pathology reports, imaging reports, medication lists
Facilitate and coordinate appointments, within established service standards, for patients requiring biopsy procedures.
Audit short term follow up exams (ACR 3) to ensure compliance
Performs other related coordinator and navigator duties as assigned.
