About The Position

Montefiore Bronx Health Collective is a Federally Qualified Health Center located in the South Bronx offering innovative, comprehensive primary care to communities challenged by social inequities and health disparities. We provide primary care services to children and adults; prenatal and reproductive health care; specialty referral management; integrated behavioral health; mental health counseling; case management and entitlements assistance; and health education. Bronx Health Collective seeks a mission-driven Women’s Health Nurse Practitioner to join our Resiliency Initiative, an evidence-based program supporting low-income families from pregnancy through early childhood. The initiative includes screening and follow-up on social drivers of health; group prenatal and pediatric care through 18 months; early childhood literacy promotion; and trauma-informed services during pregnancy and postpartum. Published outcomes demonstrate sustained healthy behaviors, longer breastfeeding duration, healthier toddler weight, improved parenting, and increased early education enrollment. The WHNP will provide comprehensive prenatal and postpartum care and reproductive health services, including contraceptive counseling and long-acting reversible contraception (LARC) placement. The role includes participation in group prenatal care and collaboration within a multidisciplinary team to advance trauma-informed care during pregnancy and postpartum.

Requirements

  • Current New York State license and registration as a Registered Professional Nurse (RN).
  • Certification and current registration in New York State as a Nurse Practitioner in the Women’s Health specialty (WHNP).
  • Completion of a graduate-level Women’s Health Nurse Practitioner program (Master’s or Doctoral degree) registered by NYSED or determined equivalent.
  • Current national board certification as a Women’s Health Nurse Practitioner (e.g., WHNP-BC).
  • Compliance with New York State collaborative practice requirements, including practice agreement provisions if within the first 3,600 hours of NP practice.
  • Strong written and verbal communication skills.
  • Proficiency with electronic health records and data systems.

Nice To Haves

  • Experience providing prenatal care, reproductive health services, and LARC placement preferred.
  • Spanish language proficiency preferred.
