About The Position

Join Community Integrated Health Services (CIHS) as a WISe Care Coordinator, supporting youth and families in the community as they build stability, independence, and a better quality of life. This is hands-on, real-world impact work—no two days are the same. You’re not behind a desk—you’re out in the community making real change. Focus on long-term stability, not quick check-ins. Team-based approach (you’re supported, not on your own).

Requirements

• High school diploma or equivalent
• 2+ years of experience working with individuals with mental health needs
• Ability to obtain WA Agency Affiliated Counselor within 7 days
• Valid driver’s license & reliable transportation
• Comfortable working in community settings

Nice To Haves

• Bachelor’s degree in Social Services or related field
• Experience in behavioral health or criminal justice settings

Responsibilities

• Work directly with youth and families in the community (homes, appointments, daily living settings)
• Help clients access benefits, healthcare, and community resources
• Partner with a multidisciplinary WISe team
• Track progress and complete documentation
