Cornerstone Family Healthcare is seeking a WHIN Peer Navigator to join their team. This part-time grant-funded position (up to 14 hours per week, grant expires July 2030) involves delivering peer-driven services to program participants in Rockland, Dutchess, Orange, and Westchester counties. The program aims to increase awareness of HIV/HCV/STI status, address Social Determinants of Health (SDOH) through educational workshops and outreach, and refer high-risk young Women of Color (WoC) for testing, treatment, and behavior modification to lower infection risk. The role is based in the Newburgh office and requires some evening and weekend hours. Cornerstone Family Healthcare is a non-profit Federally Qualified Health Center dedicated to providing high-quality, comprehensive primary and preventative health care services to all individuals, with a focus on the underserved and those without access to healthcare.
