The ACT Team is a multi-disciplinary program that provides evidence-based mental health treatment to promote the recovery and community integration of individuals with severe mental illness and/or mental health conditions and co-occurring substance use disorders. The ACT Team promotes self-determination, respects the person receiving ACT as an individual in his or her own right, and engages peers in promoting hope that the individual can recover from mental illness and regain meaningful roles and relationships within the community. The Wellness Specialist provides individual and group services, along-side of a team of mental health specialists and providers in order to assist individuals with maintaining their wellness and achieving their goals. This position is responsible for ensuring the provision of all casework services, in accordance with The ACT Team, such as providing wellness management services, and assisting with linkage to health, entitlement, educational, housing, and vocational programs to ensure persons served successful placement into the community.
