About The Position
St. Vincent's Hospital Westchester, a division of SJMC, is seeking an experienced therapist for the Wellness Specialist position in our Assertive Community Treatment (ACT) Program. Based in Harrison, NY, the ACT Program is a licensed, mobile treatment program for adults with mental illness whose needs have not been well met by more traditional programs. ACT is grounded in a recovery-oriented practice and provides an integrated set of evidence-based services delivered by a mobile, multidiscipline behavioral health treatment team.
Requirements
- NYS licensed Social Worker, Mental Health Counselor or Psychologist
- At least one year experience working with adults in an outpatient mental health setting.
Nice To Haves
- Experience working in a mobile, community-based program is preferred.
Responsibilities
- Develops individualized wellness plans with our clients and their families, utilizing the evidence-based Wellness Self-Management model.
- Works with clients to integrate self-care and healthy habits into their daily lives, emphasizing the connection between physical and mental health.
