About The Position

Cogir Senior Living is seeking a dedicated Wellness Nurse (LVN) to join their team in Memory Care. This role is crucial in ensuring the health and well-being of residents by providing high-quality care, including health assessments, medication management, and coordination with residents, families, and the care team. The Wellness Nurse will monitor resident health, address changes in condition, train and supervise staff, and foster a safe and supportive environment aligned with the company's mission of exceptional care. Requirements

A valid, incumbent nursing license (LVN) is required.
High School Diploma or equivalent.
Current First Aid and CPR license or ability to obtain.
At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies, is required, preferably in a memory care setting.
Knowledge of dementia and Alzheimer’s care is required.
Excellent attention to detail and high integrity.

Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
Ability and willingness to participate in on-call rotation.
Must meet all mandatory health requirements according to state regulations.

Nice To Haves

An associate degree or higher in nursing is preferred, or a combination of education and experience is required.
Additional coursework in management is an asset.
CDP certification is a plus or equivalent to experience and formal dementia training.
At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.

Responsibilities

Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
When necessary, participate in hands-on care, medication administration, and specific nursing tasks.
Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
Assist in the transcription of physicians' orders and the review of new physicians' orders.
Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
Coordinate care planning with home health agencies on-site, working within community and company policies.
Address residents’ special needs, requests, and complaints.
Coordinate admissions with the sales team, including supervising move-ins.
Understand the community's care regulations along with state-specific regulations.
Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
