About The Position

Stroudwater Lodge is hiring a per-diem RN or LPN Wellness Nurse for its assisted living community. The nurse provides resident-focused care, conducts health assessments, supports personalized care planning, administers and monitors medications, maintains accurate records, responds to medical emergencies, communicates with residents’ families, and advocates for residents’ rights and preferences. The role contributes to helping residents live their lives to the fullest.

Requirements

Hold a valid, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
Communicate effectively with residents, families, and healthcare professionals, and explain medical information clearly.
Analyze situations, make sound decisions, and provide appropriate solutions in a timely manner.
Demonstrate high ethical standards, respect residents’ privacy, and maintain professionalism.
Have strong record-keeping skills.
Provide care with kindness and empathy.
Complete criminal background screening.
Receive an annual influenza vaccine.

Responsibilities

Collaborate with a team to develop, implement, and review personalized resident care plans.
Conduct health assessments and identify medical concerns or changes in residents’ health.
Work with other healthcare professionals to create effective care plans.
Administer prescribed medications accurately and in accordance with protocols.
Monitor medication effectiveness and address side effects promptly.
Maintain accurate records of medical information, care plans, and incidents for regulatory compliance and continuity of care.
Respond to medical emergencies and provide immediate care, coordinating with emergency services when needed.
Communicate with residents’ families about their loved ones’ health and changes in condition.
Advocate for residents’ rights and preferences.
